Heat waves, cold snaps, wildfire smoke, hurricanes, floods, allergen blooms, disease outbreaks. The plan generally knows who is medically vulnerable and where they live. What it cannot do is reach them inside the window when contact still changes the outcome — before the dialysis appointment is missed, before the oxygen concentrator fails, before the ED visit.

Tens of thousands of at-risk members, seventy-two hours, and a call center already at capacity. The response arrives after the fact, and the plan learns who was in danger by reading the claims.

These are among the most preventable events a plan pays for, at roughly $1,700 per avoided ED visit. One heat-wave activation reached more than 16,000 members in 24 hours; 10% reported a change that likely avoided an ED visit, approximately $2M. Every activation also leaves behind a registry of medically vulnerable members, so the next one starts faster.

Surge capacity has not previously existed in member outreach. And the call resolves rather than warns: it rebooks the dialysis slot, coordinates the concentrator delivery, and initiates the emergency fill with the pharmacy.
